Many patients come to me believing a chronic diagnosis means there's nothing that can be done — that this is simply something they have to endure. That's rarely true.

While chronic conditions may not always be curable, there is almost always something we can do. We can manage your symptoms, reduce your risk of complications, and focus on what matters most — staying active and doing the things you enjoy, keeping up with grandkids, and maintaining your independence for longer.

Chronic disease management isn't about chasing numbers on a test result. It's about understanding your life, your priorities, and building a plan that works for you.

You deserve to know what you're taking and why

One of the most common things I hear is: "I'm not sure what half my tablets are for." If that sounds familiar, you're not alone — and it's something we can absolutely work through together.

I'll take the time to go through your medications with you — what each one does, why it matters, and how to take it in a way that fits your life. Where helpful, I can also refer you to a pharmacist for a thorough medication review, so nothing gets missed.

GP Chronic Disease Management Plan

If your condition warrants it, I can prepare a GP Chronic Disease Management Plan — a Medicare-supported plan that coordinates your care and gives you access to Medicare-rebated allied health sessions.

This can include support from a range of allied health professionals depending on what your condition requires.

Allied health including dietitians, physiotherapists, exercise physiologists, and podiatrists — coordinated around your individual needs.

Not everyone needs a formal plan, but for patients with complex or multiple conditions, it can make a real difference. We'll discuss whether it's right for you.

Time to cover what matters

Chronic disease management takes time to do properly. Appointments are longer by design — so we can go through your history, review your medications, talk about how you're actually feeling, and build a plan that makes sense for your life. You won't feel rushed.

Medicare rebates apply and are returned same day in most cases. Confirm current fees at the time of booking.

Telehealth is available for existing patients who have attended in person within the past 12 months, or who are registered via MyMedicare. New patients need to attend in person first.
